29 CFR 1910.23(b)(1): Every wall opening from which there was a drop of more than 4 feet was not guarded.          a) In the upper pattern storage area, on or about 8/13/14: A wall opening used to transfer pattern pieces from the upstairs storage area to the ground level production area was not guarded when actual transfer of pieces was occurring. Employees were exposed to a fall of 10 feet.
