29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch wide:    a) For employees working at 3500 South Tamiami Trail in Sarasota, as observed on or about September 17, 2014. Employees were using a tubular frame scaffold to gain access to the upper work area that was not fully planked on all working levels. The level the employees were working from was missing one plank and the opening was approximately 9 1/2 inches wide.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:   a) For employees working at 3500 South Tamiami Trail in Sarasota, as observed on or about September 17, 2014. Employees were using a tubular frame scaffold to gain access to the upper work area that did not have base plates or mud sills installed. Tubular frame scaffolding was bearing directly onto the sand.

Employees were using a step ladder that was set atop the scaffolding decking to increase the working height of the scaffold to gain access to the upper working platform.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:   a) For employees working at 3500 South Tamiami Trail in Sarasota, as observed on or about September 17, 2014.  Employees were accessing the upper work area where the point of access elevation change was greater than 19 inches and no stairway or ladder was provided. Change in elevation measured approximately 25 1/2 inches.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) For employees working at 3500 South Tamiami Trail in Sarasota, as observed on or about September 17, 2014.  The ladder the employees were using to access the upper work surface was not tall enough to allow the side rails to extend a minimum of 3 feet above the upper landing surface. The ladder was approximately 25 1/2 inches below the upper landing platform.

Employees were using the top and top step of a step ladder to gain access to the upper work overhang. The top of the ladder to ground height measured approximately 9 feet 6 inches.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about September 17, 2014, at the above addressed job site, an employee was working on the overhang of the building under construction, without the use of fall protection, exposing the employee to a fall hazard of approximately 11 feet 7 inches.  The Honors Contractors, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 974320, citation number 1, item number 2 and was affirmed as a final order on 9/24/2014, with respect to a workplace located at 6770 Forest Hills Blvd. Greenacres, FL. 33413.  The Honors Contractors, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 316417963, citation number 1, item number 2 and was affirmed as a final order on 4/24/2012, with respect to a workplace located at West Swan Ave and Henderson Blvd. Tampa, FL. 33609.
