OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  F/V Pavlof, Delta Western Dock; on or about 7 February 2015, and times prior thereto; two (2) instances were noted where electrical hazards existed:           (a) Forward Generator Room:  A 208 volt electrical panel missing a blank      (b) Forward Generator Room:  A 120 volt electrical wiring exposed taped together with black electrical tape.           These conditions exposed employees to electrical shock and electrical burn hazards.

29 CFR 1915.73(g): Gratings, walkways, and catwalks, from which sections or ladders have been removed, were not barricaded with adequate guardrails.  F/V Pavlof, Delta Western Dock, Factory area; on or about 7 February 2015, and times prior thereto; the grating was removed from around the fish sorter conveyor.  This condition exposed employees to a 22' by 10" hole in the floor measuring approximately 11 inches resulting in a tripping hazard.

29 CFR 1918.11(a): The employer used the vessel's cargo handling gear without ascertaining that the vessel had a current and valid cargo gear register and certificates in accordance with ILO 152 recommendations.     F/V Pavlof, Delta Western dock, Main deck: On or about 7 February 2015 and times prior thereto, the vessel cargo crane did not have an annual marine crane certification accomplished.  This condition exposed employees to a struck by hazard.

29 CFR 1918.35: Employees were working around open weather deck hatches which were not protected by coamings to a height of 24 inches (0.61 m), or by taut lines or barricades at a height of 36 to 42 inches (0.91 m to 1.07 m) above the deck.   F/V Pavlof, Delta Western Dock, Forward Generator Room; on or about 7 February 2015, and times prior thereto, a 32" x 37" hatch was open with no guarding.  This condition exposed employees to a 10 foot fall hazard.

This condition exposed employees to a struck by hazard.
