29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  Engine Space:  On or about 2/7/2015, and times prior thereto, employees working in the engine space with generators running were exposed to noise at 105 dB(A) for 6 hours.  This exposed employees to hearing loss hazards.

(a)  Deck:  On or about November 7, 2015 and times prior thereto, an employee would tie a knot to the side attach point of the harness.  The connection was not in the proper place and using a knot in a rope is not an acceptable way to attach.  This exposed employee to broken bones or death if they fell with the connection in this manner.

29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use:  Vessel:  On or about February 7, 2012 and times prior thereto, employees wore full face respirators when there were ammonia leaks and the vessel had no program in place for safe use of respirators.  This exposed employees chemical hazard from not having a respirator fit properly.
