29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent).  On or about May 4, 2015, employees were exposed to fall hazards, in that the employer failed to ensure that railing barriers were in place for employees working more than four feet above the next lowest level.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids and chemical glasses.   On or about May 4, 2015, employees were exposed to struck by hazards in that the employer did not ensure the use of safety glasses by employees.
