The employer did not ensure SCBA's were inspected monthly and charged at 90% and exposed employees engaged in well servicing operations to inhalation and asphyxiation hazards.

The employer did not ensure employees performing well servicing operations wore eye protection and exposed them to struck-by hazards.     Rocker A Well Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.133(a)(1) which was contained in OSHA inspection number 1060509, citation number 1, item number 2 and was affirmed as a final order on 09/11/2015, with respect to a workplace located at 6th St behind Best Western Hotel, Post, TX.
