(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  On June 10, 2021, Employees engaged in performing well servicing operations were exposed to fall hazards in excess of four feet.

29 CFR  1910.134(g)(1)(i)(A): Facial hair that comes between the sealing surface of the facepiece and the face or that interferes with valve function;   On June 10, 2021, the employer allowed employees with facial hair that went past the corners of the mouth to perform oil and gas well servicing operations while the employer had emergency escape packs with tight fitting face pieces on-site.

29 CFR  1910.134(h)(3)(i)(B): All respirators maintained for use in emergency situations shall be inspected at least monthly and in accordance with the manufacturer's recommendations, and shall be checked for proper function before and after each use.  On June 10, 2021, employees were engaged in performing well servicing operations with SCBA escape packs onsite without current monthly inspections conducted.

29 CFR  1910.133(a)(3):The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.  On June 10, 2021, employees engaged in performing well servicing operations were exposed to struck by hazards from not having safety rated  eye protection for prescription lenses.
