29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    Compounding department, process tanks: on or about May 8, 2015, employees were accessing process tanks from the top to clean interior surfaces and structures. Employees were exposed to fall hazards of approximately 10 feet where provided personal fall arrest system was not used in reliable condition to prevent fall.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment:    Compounding department, process tanks: on or about May 8, 2015, employees were accessing process tanks from the top to clean interior structures. Employees were exposed to fall hazards of approximately 10 feet because they were not trained in proper selection and use of personal fall arrest system including its limitations, proper care, and maintenance.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
