29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.    a. At the jobsite, employees were exposed to a 13'-9" fall hazard, in that, fall protection training was inadequate. The employer allowed the employees to work from heights above 6 feet with a top rail, but no midrail in place without any other form of fall protection. In addition, the employer did not train the employees on how to inspect guardrail systems to make sure that they would be able to prevent falls. Violation was discovered on or about May 13, 2015.
