29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:       On or about July 23, 2015, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances:       1) In the die cutting and box folding area, die cutting and box folding machines was operated without guards provided to protect employees contacting moving and rotating parts.       2) Instance deleted      3) At the double backer machine, the machine was operated without guard provided to protect employees from contacting pinch points.     Employees were exposed to possible amputation, crushing, and fall hazards.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:       On or about July 23, 2015, and at times prior thereto, in the maintenance shop the employer allowed employees operating abrasive wheel grinding machine which did not have tongue guards provided.  Employees were exposed to flying fragments in the event of an exploding wheel.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:       On or about July 23, 2015, and at times prior thereto, at the gas storage area oxygen and acetylene cylinders were found stored together without a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.  Employees were exposed to the hazard of fire.
