29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    On or about December 03, 2025, at Pratt Industries, Love Box Company, located at 5700 North Hydraulic, Park City, Kansas, the employer failed to protect employees from struck-by and crushing hazards. The employer failed to adequately develop written lockout/tagout procedures for employees to follow while servicing the Bobst 924 - Cardboard folding machine. The equipment specific lockout/tagout procedures did not include:  (A) A listing of all energy sources including but not limited to pneumatic, electrical, and gravity. The employer did not document the counterweight being stuck as a gravity energy source in the machine specific lockout/tagout procedures. (B) The specific location of all listed energy sources. The counterweight was not identified in the lockout/tagout specific documentation as a energy source.  (C) The specific devices to be used to de-energize each energy source including but not limited to locks, chains, pins and other lockout devices, etc. The employer did not provide additional equipment to secure the counterweight stored energy in the vertical position during maintenance.   (D) The specific procedures for the temporary removal or transfer of lockout/tagout devices for the purpose of testing or positioning, or clearing jams. The employer did not provide guidance  on how to properly manage the counterweight sticking in an elevated position. (E) The specific steps to test and verify the effectiveness of the energy control devices and energy control measures prior to beginning or re-engaging in work.
