29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:  a) Madison Farms Construction site, building F - On or about October 1, 2015, workers used stairways having 19 risers and without handrails to access third floor work areas.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not provided and used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) Madison Farms construction site, building F - On or about October 1, 2015, employees using pneumatic nail guns to frame the structure did not wear and were not provided protective eyewear by the employer.  Simple Construction, Inc., was previously for a violation of this Occupational Safety and Health Standard or it's equivalent standard 1926.102(a)(1), which was contained in OSHA Inspection number 1042949, Citation Number 1, item number 2B, and was affirmed as a final order on May 27, 2015, with respect to a workplace located at 3 Mary Lane, block 26, Lot 2 Robbinsville, NJ 08691.
