29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  On or about December 9, 2015, at the south, east and west of the residential building, three employees using air nail guns to install roof shingles were not wearing eye protection, exposing themselves to being struck by flying debris or nails.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a.  On or about December 9, 2015, at the south side of building #11, employees removing shingles and installing weather proofing material on a 5:12 pitch roof were not protected from open holes by the use of a fall protection system.  b.  On or about December 9, 2015, at the southeast side of building #12, three employees installing weather proofing material on a 5:12 pitch roof were not protected from open holes by the use of a fall protection system.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about December 9, 2015, at the north and east side of building #6, six employees installing roof shingles on a 5:12 pitch residential roof were not protected from an 18ft fall hazard by the use of a fall protection system.  b.  On or about December 9, 2015, at the south side of building #11, eight employees removing shingles and working on the leading edge of a 5:12 pitch roof were not protected from an 18ft fall hazard by the use of a fall protection system.  c.  On or about December 9, 2015, at the southeast side of building #12, three employees installing weather proofing material on a 5:12 pitch roof were not protected from an 18ft fall hazard by the use of a fall protection system.
