29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a)  Sub Arc Welder (SAW) #1 (Closest to Offices); On or about January 13, 2016 and at times prior, the employer exposed employees to electric shock hazards in that the employees were allowed to turn off the power at the operators control panel when changing welding heads, in lieu of doing a complete lockout/tagout at the power panel.  b)  Sub Arc Welder (SAW) #2 (Between SAW 1 and SAW 3); On or about January 13, 2016 and at times prior, the employer exposed employees to electric shock hazards in that the employees were allowed to turn off the power at the operators control panel when changing welding heads, in lieu of doing a complete lockout/tagout at the power panel.  c)  Sub Arc Welder (SAW) #3 (Between SAW 3 and SAW 4); On or about January 13, 2016 and at times prior, the employer exposed employees to electric shock hazards in that the employees were allowed to turn off the power at the operators control panel when changing welding heads, in lieu of doing a complete lockout/tagout at the power panel.  d)  Sub Arc Welder (SAW) #4 (Furthest from Offices); On or about January 13, 2016 and at times prior, the employer exposed employees to electric shock hazards in that the employees were allowed to turn off the power at the operators control panel when changing welding heads, in lieu of doing a complete lockout/tagout at the power panel.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed.  a)  Sub Arc Welder (SAW):  On January 13, 1016 and at times prior, the employer exposed employees to electric shock hazards in that a periodic inspection of the machine specific procedures for the Sub Arc Welder (SAW) were not conducted annually.

29 CFR 1910.147(d)(1):  Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy.  a)  Pipe Mill Hydraulic Kickers Zone 1; On January 13, 2016 and at times prior, the authorized employee did not have knowledge of the type and magnitude of the energy, in that the machine specific procedures did not list the magnitude of the hydraulic pressure.
