29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by points of operation:    a)  Pipe Kicker at Coating Mill Outbound Deck;  On or about March 10, 2016 and at times prior thereto, the employer exposed employees to struck by and crushed by hazards in that the employer did not have guarding in place to ensure employees working on the deck were protected from pipes that were being mechanically rolled onto the deck.
