OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from crushing and/or struck by hazards: (a) Facility, 234 Lott Road, Pittsburgh, PA: On or about 03/28/16 the SK 2000 Steel King Industries pallet racks used to store sheets of aluminum and other products were not anchored to the floor in accordance with the manufacturer's instruction. The racks are loaded by means of powered industrial equipment thereby creating a potential tip-over and/or crushing hazards should the racks be struck during loading. Employees work in the immediate area of the racks and have the potential of being struck by the racks should a tip-over occur. In addition to the Steel King racks there are numerous rack systems of non-descript manufacturing used in the facility to hold molds and other supplies that are not bolted to the floor to prevent movement.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Facility, 234 Lott Road, Pittsburgh, PA: On or about 03/28/16 the employer had not established a program that consisted of energy control procedures, employee training and periodic inspections. Employees perform servicing and maintenance on equipment such as CNC machines, shears, press brakes, mold machines and saws and the employer does not have a program to ensure employees understand machine energies and isolating controls and how to control equipment for protection from unexpected startup or release of stored energy.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Facility, 234 Lott Road, Pittsburgh, PA: On or about 03/28/16 the foot controlled Accur-Shear 10 ton shear used by employees to cut metals and cardboard did not have an adequate guard to protect the employees from the point of operation during cycling. Employees cut metals and cardboard with thickness(s) of 1/8" from sizes of 10' down to 4 1/2" and the opening in the guard measured 2 1/4" allowing access to the point of operation. (b) Facility, 234 Lott Road, Pittsburgh, PA: On or about 03/28/16 the foot controlled Accur-Press 60 ton brake used by employees to bend metal(s) did not have any guards in place to prevent the operator from having any body part in the danger zone during operating cycles. Employees bend metals 2" x 8' into 1" x 8' and there is no guard in place to prevent the operator from contacting the point of operation.

The employees have not reviewed the safety data sheets to ensure they understood the potential hazards associated with the products being used and what types or methods of personal protection should be used when working with such products.
