OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from crushing and/or struck by hazards:    (a) Facility, 234 Lott Road, Pittsburgh, PA:  On or about 03/28/16 the SK 2000 Steel King Industries pallet racks used to store sheets of aluminum and other products were not anchored to the floor in accordance with the manufacturer's instruction.  The racks are loaded by means of powered industrial equipment thereby creating a potential tip-over and/or crushing hazards should the racks be struck during loading.  Employees work in the immediate area of the racks and have the potential of being struck by the racks should a tip-over occur.    In addition to the Steel King racks there are numerous rack systems of non-descript manufacturing used in the facility to hold molds and other supplies that are not bolted to the floor to prevent movement.

(b) Facility, 234 Lott Road, Pittsburgh, PA:  On or about 03/28/16 the foot controlled Accur-Press 60 ton brake used by employees to bend metal(s) did not have any guards in place to prevent the operator from having any body part in the danger zone during operating cycles.  Employees bend metals 2" x 8' into 1" x 8' and there is no guard in place to prevent the operator from contacting the point of operation.
