29 CFR 1910.95(b)(1): Employees were subjected to sound levels exceeding those listed in Table G-16 of Subpart G of 29 CFR 1910 and feasible administrative or engineering controls were not utilized to reduce sound levels:    a) In the facility, on or about May 29, 2018 - An employee shaking out aluminum castings on the Herschal Knockout Machine was exposed to continuous noise at 1,931% of the permissible daily noise exposure (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 111 dBA during the 182 minute sampling period.  Zero exposure was assumed for the 298 minutes that were not sampled.

29 CFR 1910.95(d)(3)(ii): Repeat employee exposure monitoring was not performed when hearing protection attenuation was inadequate in meeting the provisions of 29 CFR 1910.95(j):  a) In the facility, on or about May 29, 2018 - The employer did not repeat noise monitoring whenever a change in production, process, equipment or controls increased noise exposures to the extent that the attenuation provided by hearing protectors used by employees may be rendered inadequate to meet the requirements of 29 CFR 1910.95(j).  Hearing protection utilized by employees operating the Herschal Knockout Machine did not attenuate exposure at least to an 8-hour Time-Weighted Average (TWA) of 90 decibels.

29 CFR 1910.95(j)(2): Hearing protectors did not attenuate employee exposure at least to an 8-hour time-weighted average of 90 decibels as required by 29 CFR 1910.95(b):  a) In the facility, on or about May 29, 2018 - Hearing protectors provided to employees did not attenuate employee exposure at least to an 8-hour time-weighted average of 90 decibels as required by 29 CFR 1910.95(b).  An employee shaking out aluminum castings on the Herschal Knockout Machine was exposed to continuous noise at 1,931% of the permissible daily noise exposure (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 111 dBA during the 182-minute sampling period.  The hearing protectors utilized by employees provided a NRR of 24 dB or an estimated 94 dBA under the ear protector.
