29 CFR 1960.8(a): The head of each agency did not furnish to each employee employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm from struck-by and caught in between hazards:     At the workplace on or about May 24, 2016, the employer allowed employees to conduct maintenance activity on a KC-135 aircraft without following the maintenance procedures as required by each applicable Technical Order(s) and Process Order(s)for the aircraft.  An employee was exposed to the hazards of struck-by and caught in between.     1)      Rudder control system functional check out was not accomplished in accordance with TO 1C-135-2-8-1 Chapter 18, PO 76 AMXG 00-20, PO 76 AMXG 00-13, and PO 76 AMXG 11-01 as assigned by Work Control Document (WCD) tracking #RT1662720264.   2)      Close out inspection and install of the tail cone was not being accomplished in accordance with TO 1C-135-2-2-2 Chapter 16, PO 76 AMXG 01-30 and PO 76 AMXG 11-01 when the incident occurred on May 24, 2016 as assigned by Work Control Document (WCD) tracking #RT1662764018.  3)      Hydraulic crew assist BA skill to rig L/H & R/H aileron control system cables. Note: Split flaps for rigging was not being accomplished in accordance with TO 1C-135-2-8-1 Chapter 14 and PO 76 AMXG 11-01 when the incident occurred on May 24, 2016 as assigned by Work Control Document (WCD) tracking #RT1662729647.
