On or about February 10, 2019, in building 3001, buckets, brooms, and dust pans used to clean thermal spray booths were not covered or enclosed to contain thermal spray debris when the equipment was brought out of thermal spray booths. Employees were exposed to thermal spray debris which contained hazardous chemicals including but not limited to cobalt, nickel, and chromium which were irritants, sensitizers, and/or suspect carcinogens. b. On or about April 11, 2019, in building 3001 dust on elevated surfaces near the mezzanine above thermal spray booths, was not removed and contained hazardous chemicals including but not limited to Cobalt, Nickel, and Chromium which were irritants, sensitizers, and/or suspect carcinogens.
