29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   At the jobsite, on or about May 23, 2016, the employer allowed an employee to operate a hydraulic shear without having the point of operation guard properly applied and adjusted.  The employee was exposed to the hazard of amputation.

29 CFR 1910.212(a)(3)(iii): Special handtools for placing and removing material to permit easy handling of material without the operator placing a hand in the danger zone were not provided:        At the jobsite, on or about May 23, 2016, the employer allowed an employee to operate a hydraulic shear without providing special handtools to prevent the operator from placing his hand in the point of operation area.  The employee was exposed to the hazard of amputation.
