29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function:     On or about August 18, 2016 employees were not protected from a fall as the safety monitor was engaged in roofing activities, exposing employees to a fall of 13.1 feet.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
