Guard door rails had been removed to make room for the RETROFLEX, INC Print Section which was added to the top deck at the end of November, 2016.    b)    On or about 1/12/2017, Guard door micro-switches were not working to shut down Machine #7 in Bay #5 when the doors were opened to test the micro-switches for: Folding cylinder slider guard #5 on Machine #7, Out-feed guard door on North side of Machine #7, Back door out-feed guard on South side of Machine #7, and Band saw blade door guard on Machine #7.    c)    On or about 1/12/2017, Nip guards were needed on draw rolls for Machine #1.    d)    On or about 1/12/2017, Packer fingers nip point guarding was missing on the backside of Machine #1.    e)    On or about 1/12/2017, Slitter guard was missing on the top of Machine #1.    The Swanson Wiper Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1) which was contained in OSHA inspection number1103331, citation number 1, item number 2 and was affirmed as a final order on 2/17/2016 with respect to a workplace located at 645 Leeward Court West, Oshkosh, Wisconsin, 54901.
