29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 5/14/2018, Superior Cartoner machine #5 had interlocked plexi-glass guard doors that were not wired to shut the machine down if employees opened them to clear jambs while the machine was running exposing them to struck by or caught in hazards. The Swanson Wiper Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1) which was contained in OSHA inspection number 1201938, citation number 1, item number 1 and was affirmed as a final order on 3/22/2017 with respect to a workplace located at 645 Leeward Court West, Oshkosh, Wisconsin, 54901. 29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: a) On or about 5/14/2018, Superior Cartoner machine #5 had an opening approximately 19" high by 14" wide which was not guarded exposing chains and sprockets in an area where operators had to adjust flights for the out feed end of the conveyor occasionally exposing them to caught in and struck by hazards. The Swanson Wiper Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(f)(3) which was contained in OSHA inspection number 1103331, citation number 1, item number 3/d and was affirmed as a final order on 2/17/2016, with respect to a workplace located at 645 Leeward Court West, Oshkosh, Wisconsin, 54901.
