29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about 3/29/17 located at 875 Grove Glen Cir, Sugar Hill, GA:  Employees were exposed to fall hazards from 4 to 10-feet from the lower level, while performing framing work from the make shift scaffolds made from 2 X 6 and 2 X 10 lumber. The employer failed to install guard rails

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 3/29/17 located at 875 Grove Glen Cir, Sugar Hill, GA., Employees were observed and exposed to fall hazards, while framing a new house, employees were using a portable extension ladder to gain access to the 2nd floor, and the ladder was only 13- inches above the landing surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about 3/29/17 located at 875 Grove Glen Cir, Sugar Hill, GA: Employees were installing plywood on the roof of a house that was under construction while working at the edge. The employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately from 21 to 30- feet with a 7/12 and 10/12 pitch.
