29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     (a) Shaver Transportation - Shop Barge - On or about June 8th, 2017 and at times prior thereto the CSHO observed a horizontal bandsaw without adequate guarding of the unused portion of the blade.

29 CFR 1915.73(d): When employees are exposed to unguarded edges of decks, platforms, flats, and similar flat surfaces, more than 5 feet above a solid surface, the edges shall be guarded by adequate guardrails meeting the requirements of 1915.71(j)(1) and (2), unless the nature of the work in progress or the physical conditions prohibit the use or installation of such guardrails.    (a) Shaver Transportation - barge ST-34 and shop barge - On or about June 8th, 2017 the employer exposed employees to unguarded edges of the shop barge while employees transitioned to barge ST-34.      Shaver Transportation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1915.73(d), which was contained in OSHA inspection number 996883, citation number 1, item number 1 and was affirmed as a final order on 3/13/2015, with respect to a workplace located at 4900 N.W. Front Ave Portland, OR. 97210.
