29 CFR 1910.28(b)(3)(v)(B): Each employee was not protected from falling through a chute-floor hole by a removable guardrail system and toe-boards on not more than two sides of the hole and a fixed guardrail system on all other exposed sides.  a) Production Area- Outside of Viatera I Ring Mixing Room: On or before October 10, 2017, and at times prior, the employer exposed employees to fall hazards, in that the employer did not provide a covering to keep employees from falling onto the next lower level through an unguarded chute-floor hole opening.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  a) Production Area- Viatera I Ring Mixing Room - On or before October 10, 2017, and at times prior, the employer exposed employees to caught-in hazards in that the employer did not ensure that employees utilized lockout procedures by isolating the electrical energy sources when performing cleaning tasks on the Ring Mixer.
