29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous condition:   a) Construction site, 1341 Memorial Drive, roof - On or about June 7, 2018, two carpenters installing roof sheathing were using pneumatic nail guns and a circular saw to conduct the operation but were not wearing protective eyewear.   b) Construction site, 1341 Memorial Drive, ground level -  On or about June 7, 2018, a carpenter using a DeWalt circular saw to cut pre cut roof sheathing was not wearing protective eyewear.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) Construction site, 1341 Memorial Drive, roof - On or about June 7, 2018, two carpenters installing roof sheathing were not protected from falling. The carpenters had not transitioned to conventional fall protection in accordance with their written 502(k) plan when enough sheathing had been installed to allow for proper anchorage.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Construction site, 1341 Memorial Drive, rear eave of roof - On or about June 7, 2018, employees used an aluminum extension ladder that did not extend 3 feet above the landing surface to gain access to the roof top work area.
