29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a) 5102 Rochelle Ave., Philadelphia, PA 19128 - The employer permitted employees to ascend and descend a self-supporting ladder that was in the closed position to access a landing approximately 12 feet above the lower level, thus exposing employees to fall from height hazards.  This condition was noted on August 23, 2019.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 5102 Rochelle Ave., Philadelphia, PA 19128 - The employer did not ensure that employees who were performing framing operations at heights up to approximately 48 feet above the lower level were protected by guardrail systems, safety net system, personal fall arrest system, or an alternative fall protection measure, thus exposing employees to fall from height hazards.  This condition was noted on August 23, 2019.  Best Quality Construction Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1390837, citation number 1, item number 2, and was affirmed as a final order on June 24, 2019, with respect to a workplace located at 2321 South 4th Street, Allentown, PA 18103.  Best Quality Construction Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1321576, citation number 1, item number 4, and was affirmed as a final order on September 17, 2018, with respect to a workplace located at Woodlands At Warwick, 1341 Memorial Dr., Warwick, PA 18974.
