29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section.   Location: 2 Lincoln Ave., Lake Hiawatha, NJ.  a.) Employees were exposed to amputations and other hazardous conditions when utilizing a power hand tool that had an abrasive wheel attached while not being equipped with the hand tool's protective guards; on or about 10/5/18.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  Location: 2 Lincoln Ave., Lake Hiawatha, NJ.  a.) Employees were exposed to electrical hazards when utilizing electric powered hand tools that were energized by an extension cord without a ground fault circuit interrupter, as well as not providing the employees with an assured equipment grounding conductor program; on or about 10/5/18.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:    Location: 2 Lincoln Ave., Lake Hiawatha, NJ.    a) An employee engaged in paneling activities used a JLG aerial lift without the use of a personal fall arrest system.  Employees were using the aerial lift to install panels and wood strips on the face of the building; on or about 10/5/18.
