29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) SeWyCo Social Hall, Kenny Headman Lane, Bethlehem, On or about July 31, 2019, employees were performing roofing work on a steeply pitched roof, approximately 10 feet above grade, without utilizing any means of fall protection.  Loft Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1376862, citation number 1, item number 1A and was affirmed as a final order on June 17, 2019, with respect to a workplace located at 1701 Howard Avenue, Philadelphia, PA.  Loft Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1372603, citation number 1, item number 1 and was affirmed as a final order on May 1, 2019, with respect to a workplace located at Stevensan Drive, Sewickley, PA..

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  a) SeWyCo Social Hall, Kenny Headman Lane, Bethlehem, On or about July 31, 2019, employees were performing roofing work while accessing the roof from a ladder that did not have its side rails extended at least three feet above the upper landing surface being accessed.
