Front St., Philadelphia, PA 19147 - The employer did not certify that an employee who operated a Pettibone Model No. 10044 telehandler powered industrial truck had been trained and evaluated, thus exposing employees to struck-by and caught-in/between hazards. This condition was observed on August 3, 2019.
