29 CFR 1910.178(l)(2)(i)(A): The employer did not ensure trainees operated a powered industrial truck only under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence: a) Shipping Department, Finished Goods Cooler - On or about September 11, 2019 and times prior thereto, the employer did not ensure that each trainee who operated a Crown Narrow-Aisle RR 5700 Series stand-up forklift did so only under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence. b) Shipping Department, Finished Goods Cooler - On or about September 11, 2019 and times prior thereto, the employer did not ensure that each trainee who operated a Crown WP 3000 Walkie Pallet Truck did so only under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
