29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Meat Room - The RMF Steel Products Mixer/Blender, model 28-300, serial number 26383, had energy sources to include electric and pneumatic.  The employer's written machine-specific procedure for this piece of equipment did not include pneumatic energy and so did not include steps for how to de-energize and lockout/tagout the pneumatic energy, exposing employees to getting struck-by and caught-in moving machine parts, on or about October 5, 2021.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Meat Room - The discharge door guard was missing on the RMF Steel Products Mixer/Blender, model 28-300, serial number 26383, exposing employees to getting caught in or by rotating mixing blades, on or about September 24, 2021.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1904.39(a)(2): The employer did not report within 24 hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  (a) Winter Gardens Quality Foods, Inc., 304 Commerce Street, New Oxford, Pennsylvania - An employee suffered a fingertip amputation on September 24, 2021, and the employer failed to report this amputation to OSHA within 24 hours.  (The employer reported this amputation to OSHA on September 27, 2021.)  Corrected during the inspection.
