29 CFR 1910.217(c)(2)(i)(b): Barrier guards did not conform to the maximum permissible openings of Table O-10:      Instances including, but not limited to, the following:    a) Job site, 33 McGovern Boulevard, Crescent, PA: On or about December 12, 2019, during production activities at press number 808, located in building 4, the employer did not ensure that the barrier guard openings were compliant with table 0-10 in that the guard on the front of the unit had an opening size of approximately 10 inches by 6 inches and the distance to the tooling area was approximately 14 1/2 inches.

29 CFR 1910.217(e)(3): The employer did not insure the original and continuing competence of personnel caring for, inspecting, and maintaining power presses:      Instances including, but not limited to, the following:    a) Job site, 33 McGovern Boulevard, Crescent, PA: On or about December 12, 2019, during production activities in building 4, such as at press numbers 808, 805, 854 and 852, the employer did not ensure that maintenance staff were properly trained to recognize proper minimum safety practices related to press operations. Deficiencies noted during the inspection included, but were not limited to, failing to ensure the two hand controls used for machine guarding were fixed in place, not ensuring that barrier guards were properly affixed to presses, failing to adhere to maximum barrier guard opening sizes in accordance with table 0-10 and not ensuring that safety distances based on 90 degree on the downstroke stopping time measurements were obtained and used as a basis for two hand control or light curtain replacement for machine guarding purposes.

29 CFR 1910.217(f)(2): The employer did not train and instruct the operator in the safe method of work before starting work on any operation covered by this section or did not insure by adequate supervision that correct operating  procedures were being followed:       Instances including, but not limited to, the following:    a) Job site, 33 McGovern Boulevard, Crescent, PA: On or about December 12, 2019, during production activities in building 4, such as at press numbers 854, the employer did not ensure that production personnel were provided proper mechanical power press safety training. Deficiencies noted during the inspection included, but were not limited to, not providing appropriate training on the relationship between press stopping times and the placement of 2 hand control devices when used as machine guarding or that the two hand controls had to be fixed in place when in use as machine guarding.
