29 CFR 1910.217(b)(1): Machine component(s) of mechanical power press(es) were not designed, secured, or covered to minimize hazards caused by breakage, or loosening and falling, or release of mechanical energy: (a) On or about April 14, 2020, a set screw backed off to the holding hinge pin on the Toledo (TA201C) 35 ton mechanical power press. Due to this, the holding hinge pin fell out of the press and allowed the upper die to fall. When the upper die fell the operator suffered 3 broken fingers.

29 CFR 1910.217(e)(3): Maintenance personnel were not provided training by the employer to ensure the original and continuing competence of caring for, inspecting and maintaining power presses: (a) Facility: On or about June 23, 2020, the employer did not ensure that the maintenance staff was properly trained to identify, maintain, or make repair of presses. Deficiencies noted during the inspection included but not limited to, maintenance staff not properly identifying the cause of ghost cycles and missing weekly inspections.

29 CFR 1910.217(e)(1)(i): The employer did not follow a program of periodic and regular inspections of his power presses to ensure that all of their parts, auxiliary equipment, and safeguards were in a safe operating condition and adjustment: (a) Facility: On or about April 14, 2020, the employer did not inspect its Toledo (TA2010) 35 ton, part revolution mechanical power press for over a month before the incident on April 14, 2020, where an employee was injured when a set screw backed off and this allowed a steel clutch pin to fall out of the press. This allowed the top die to fall, breaking three fingers of the operator. The last weekly inspection for this press was done on March 11, 2020. This press was operated on March 24, 25, 26, 27, 2020 and on April 6, 2020 before the incident where the machine operator was injured on April 14, 2020.
