29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  On or about 1/8/2020 at 6461 Crosscreek Lane, Lot 65Q, Flowery Branch, GA where a worker was observed exposed to 7-foot fall hazards while performing framing operations from a job-made scaffold that was not inspected by a competent person prior to use where:  a. erected with a 5 and 1/2 - inch working surface; b. access and egress was accomplished with a folded 6-foot step ladder; c. support beams were resting on the stone ground cover not properly supported with mudsills; d. support beam was installed not in a continuous length and supported by the pile of lumber.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about 1/8/2020 at 6461 Crosscreek Lane, Lot 65Q, Flowery Branch, GA where a framer was observed working from a 7-foot job made scaffold utilizing a folded 6-foot step ladder for access and egress, exposing him to 6-foot fall hazards where the ladder required him to step on the top step for access to the working surface.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  Framing Solutions and Crane Services, LLC, Flower Branch, GA - On or about 01/08/2020, at 6461 Crosscreek Ln, Lot 65Q, Flowery Branch, GA, 30542, employees were exposed to eye injuries from flying objects such as wood chips, nails and sawdust when using a nail gun and cutting with a circular saw while performing framing operations without eye protection.  Framing Solutions and Crane Services, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number1206624, citation number 1, item number 2 and was affirmed as a final order on 07/19/2017, with respect to a workplace located at Lot 11, 5735 Rear Road, Sugar Hill, GA.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 1/8/2020 at 6461 Crosscreek Lane, Lot 65Q, Flowery Branch, GA where a worker was observed carrying a piece of plywood on his shoulders in breezy conditions, on the 10/12 pitched roof, exposed to 20-foot fall hazards while installing plywood sheathing without wearing personal fall protection.  Framing Solutions and Crane Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1206624, citation number 01, item number 04 and was affirmed as a final order on 7/18/2017, with respect to a workplace located at 5735 Riverside Bay, Sugar Hill, GA 30518.
