29 CFR 1910.22(d)(2): The employer did not ensure hazardous conditions on walking-working surfaces were corrected or repaired before an employee used the walking-working surface again.  When the correction or repair cannot be made immediately, the hazard was not guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired:  a) Pouring Area, grating around pressure pourer A; On or about February 11, 2019 and at times prior to, the employer failed to repair walking-working surfaces after areas had been cut out to gain access to the pressure pourer for maintenance while still allowing the area to be used as access and egress.  b) Pouring Area, upper mezzanine around pressure pourer B; On or about February 11, 2019 and at times prior to, the employer failed to repair walking-working surfaces that contained holes in the grating while requiring employees to work in the area.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet:    a) Maintenance shop; On or about February 11, 2020 and at times prior to the employer allowed oxygen bottles that were not in use to be stored at the area were acetylene torch cutting was conducted without being separated by a minimum distance of 20 feet or an approved firewall exposing the employees to explosion hazards.
