29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems.   a)  Melt Deck, Alloy Feeder Area; On or about November 28, 2022: the employer exposed employees to slip/trip hazards in that employees were allowed to walk and work around a floor hole (trash/scrap location) that was not protected by a cover or guardrail system.
