29 CFR  1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:  a) Building 143, east side, top deck, on or about 3-11-21:  Employees were standing on floor trusses, without fall protection, exposing the employees to a fall hazard of 34 feet to the ground outside of the building.  b) Building 129, east side, top deck, on or about 3-11-21:  Employees were standing on floor trusses, without fall protection, exposing the employees to a fall hazard of 16 feet to the ground inside of the building.  c) Building 129, east side, top deck, on or about 3-11-21:  Employees were standing on floor trusses, without fall protection, exposing the employees to a fall hazard of 10 feet to the wood deck below.

29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:   a) Building 143, east side, top deck, on or about 3-11-21:  One employee was standing on floor trusses, connected to a personal fall arrest system (PFAS) that was rigged with excess slack, such that the employee could fall more than 6 feet before the fall could be arrested, exposing the employees to a fall hazard of up to 34 feet to the ground outside the building and/or 10 feet to the wood deck below  b) Building 129, east side, top deck, on or about 3-11-21:  One employee was standing on floor trusses, connected to a personal fall arrest system (PFAS) that was rigged with excess slack, such that the employee could fall more than 6 feet before the fall could be arrested, exposing the employees to a fall hazard of 16 feet to the ground inside of the building.
