29 CFR  1910.304(g)(5):Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.  a. On or about August 31, 2021, located at Line 3 of the light assembly area, a metal pedestal fan was in use that was plugged into an extension cord that had a missing ground pin, thereby exposing employees to an electrical shock hazard.

29 CFR  1910.305(g)(2)(iii):Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  a. On or about August 31, 2021, located at pallet wrapping machine adjacent Lines 6 and 7, the flexible supply cord did not have an effective strain relieve at the cord plug.

29 CFR  1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:   29 CFR  1910.305(g)(1)(iv)(A): As a substitute for the fixed wiring of a structure.  a. On or about August 31, 2021, the employer used multiple extension cords to energize assembly conveyor equipment, pedestal fans, taping machines, a pallet wrapping machine, and to daisy chain multiple cords together. The cords were being used as a substitute of fixed wiring to energize the equipment.
