Dirt Department: A step from the grinding platform was not maintained in a safe manner. The step broke on February 2, 2022, while a worker was coming down from the grinder platform resulting in a injury. The step was not repaired and bags of dirt were being used to access and egress from the platform.

OSH ACT of 1970 Section (5)(a)(1):Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to rack collapse and overhead struck - by hazards while working in the warehouse: a. Main Drive Aisle: Sections 6-7 and 2-3 had damaged storage rack legs. b. Second Warehouse: Several storage racks in aisle 27 had damaged storage rack legs. The Plantation Products dba Green Garden Products was previously cited for a violation 5(a)(1), which was contained in OSHA inspection number 1228736, citation number 1, item number 1 and was affirmed as a final order on 11-6-17, with respect to a workplace located at 202 South Washington Street, Norton, MA 02766.
