29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a) Norfolk Naval Shipyard, B234 2nd floor welding lab machine shop: On or about 9/8/21, the employer assigned employee(s) to work on a lathe (SN 550125-T0070), in order to buff 1" carbon steel pipe. The buffing process required the employee to bypass the built-in chuck guard to wrap an emery cloth around the pipe and hold the emery cloth by hand as the pipe rotated, which created an unguarded point of operation between the emery cloth and rotating pipe material.  During this operation, the employee's hand made contact with the point of operation, resulting in the amputation of their right index finger.  Norfolk Naval Shipyard was previously cited for the same violation of this Occupational Safety and Health Standard OSH ACT of 1970 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1423207, citation number 1, item number 1 and was affirmed as a final order on 11-26-2019, with respect to a workplace located at Norfolk Naval Shipyard, VA.  Abatement certification required within 15 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.132(d)(1)(ii):When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not communicate the selection decision(s) to each affected employee:  a) Norfolk Naval Shipyard, B234 2nd floor welding lab machine shop: On or about 9/8/21, an employee held an emery cloth that was wrapped semi around a 1" carbon steel pipe segment fastened into the work hold device of an industrial lathe (SN 550125-T0070).  The employee was never informed about or instructed to review the lathe's associated Job Hazard Assessment (JHA), which prohibited the wearing of gloves during the lathe's operation, due to the caught-in hazard created by the rotating machinery.  The employee wore work gloves while operating the lathe, which resulted in their hand being pulled into the rotating pipe material and amputating the employee's right index finger.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
