29 CFR 1915.71(j)(1): Scaffolding, staging, runways, or working platforms which are supported or suspended more than 5 feet above a solid surface, or at any distance above water, were not provided with a railing which had a mid-rail located halfway between the top rail and the staging, platform, or runway:   a)    McFarland Dredge: On or about 03/22/2022, the competent person failed to ensure that the mid-rail was halfway between the top rail in that the top rail was erected at 39 inches and the mid-rail at 24 inches from a single plank, as a result subcontractors and employees were exposed to a possible 10ft fall hazard.   No abatement certification or documentation required.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a)     McFarland Dredge: On or about 03/22/2022, a person competent in identifying hazardous working conditions on a tube and coupling scaffolding system erected in the hopper was not assigned that could take prompt corrective measure to eliminate them.  No abatement certification or documentation required.

29 CFR 1915.81(a)(4): The employer did not maintain easy and open access to each fire-alarm box, fire-call station, fire-fighting equipment, and each exit, including ladders, staircases, scaffolds, and gangways:   a)	McFarland Dredge: On or about 03/22/2022, the competent person failed to ensure that access to the scaffold ladder was easy and open in that employees and subcontractors were required to step through or climb over an existing guardrail system to access the scaffold and ladder.  No abatement certification or documentation required.

29 CFR 1915.81(b)(1)(iv)(A): The employer did not place each hose and cord above the walkway in a location that will prevent injury to employees and damage to the hoses and cords:  a)	McFarland Dredge: On or about 03/24/2022, the subcontractor employees did not place hoses and extensions above the walkway in the hoses and extension cords used in descaling the hopper were placed on the deck in the walkway thus created a trip hazard.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
