29 CFR 1915.72(a)(3): Portable ladders shall be lashed, blocked or otherwise secured to prevent their being displaced. The side rails of ladders used for access to any level shall extend not less than 36 inches above that level. When this is not practical, grab rails which will provide a secure grip for an employee moving to or from the point of access shall be installed. Location: 400 E Indian River Rd, Norfolk, VA 23523 On 30 September 2023 and times prior to: Employees used a faulty ladder not positioned at least 36" above the point of access was used to access a transporter and was not lashed, blocked, or otherwise secured to prevent their displacement.
