This citation type is amended to:  Other-than-Serious  The SAVE is amended to:   29 CFR 1910.30(a)(3)(i): The employer did not train each employee in the nature of the fall hazards in the work area and how to recognize them:  The AVD is amended to:  (a) On or about a March 16, 2022, and at times prior, drivers were exposed to fall hazards when climbing on top of 12-foot-4-inch tank trailers without first having had adequate training on hazard recognition and hazard mitigation methods.  29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  (a) On or about March 16, 2022, and at times prior, drivers were exposed to fall hazards when climbing on top of 12-foot-4-inch tank trailers without fall protection to conduct pre- and post-trip inspections.
