Employees engaged in product return activities in the warehouse, worked in an area where the door marked as an exit was not usable as an exit, and was not marked as "Not an Exit".  Exit doors from the employer's warehouse led into another warehouse and could not be accessed as an exit.  Violation observed on or about April 20, 2022

Employees engaged in warehousing activities operated equipment that included, but was not limited to Crown, Model PE 3500 Series order pickers and Crown, Model PE 4500 Series electric pallet jacks, that were not provided formal training that included classroom instruction, a written exam, evaluation, and certification to operate the equipment. The employer authorized the installation Relocatable Power Taps (RTP's) to power equipment and plugged additional RTP's (daisy chained) to increase the number of outlets available for additional equipment in a manner that was not approved by the manufacture's listing. An employee, operating a Crown order picker was injured on April 11, 2022 and the employee did not report the hospitalization until April 18, 2022  Violation occurred on or about April 18, 2022
