29 CFR  1926.102(a)(1):Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about June 6, 2022, during the installation of roofing material at the job site located at 5948 Spring Lake Rd NW, Canton, Ohio 44718; the employer did not ensure employees utilizing an air powered nailer were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about June 6, 2022, during the installation of roofing material at a job site located at 5948 Spring Lake Rd NW, Canton, Ohio 44718; the employer did not ensure that all employees working on the roof of a single family home were protected from a fall of approximately 11 feet by utilizing a fall protection system.  Hoover Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(B)(13) Duty To Have Fall Protection), which was contained in OSHA inspection number 1566543, citation number 01, item number 01 and was affirmed as a final order on (02/22/2022), with respect to a workplace located at 4526 Massillon Rd, N. Canton, Ohio 44720.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 6, 2022 at the job site located at 5948 Spring Lake Rd NW, Canton, Ohio 44718; during the installation of roofing material on a residential roof, the employer did not ensure portable ladders extended 3 feet above the upper landing surface.    Hoover Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(B)(1) Ladders), which was contained in OSHA inspection number 1566543, citation number 01, item number 02 and was affirmed as a final order on (02/22/2022), with respect to a workplace located at 4526 Massillon Rd, N. Canton, Ohio 44720.
