29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about November 20, 2023, the employer did not provide a training program for employees exposed to a fall hazard of 10 feet above the grass covered ground while removing shingles from the roof a residential home with an 8/12 pitch located at 2834 Canton Road Uniontown, OH 44685.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about November 20, 2023, employees were working on a residential roof located at 2834 Canton Road Uniontown, OH 44685, with an 8/12 pitch, 10 feet above the grass covered ground without utilizing or wearing any form of fall protection.  Hoover Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1600238, citation number 2, item number 1 and was affirmed as a final order on 8/29/2022, with respect to a workplace located at 5948 Springlake Rd NW Canton, Ohio 44718.  Hoover Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1566543, citation number 1, item number 1 and was affirmed as a final order on 2/22/2022, with respect to a workplace located at 4526 Massillon Rd, North Canton, Ohio 44720.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about November 20, 2023, employees were exposed to a fall hazard of 10 feet while completing roofing work on a residential home located at 2834 Canton Road, Uniontown, OH 44685. Employees were using an extension ladder to access the roof and the employer did not ensure it extended at least 3 feet above the upper landing surface.  Hoover Roofing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1600238, citation number 2, item number 2 and was affirmed as a final order on 08/29/2022, with respect to a workplace located at 5948 Springlake Road NW Canton, Ohio 44718.  Hoover Roofing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1566543, citation number 1, item number 2 and was affirmed as a final order on 02/22/2022, with respect to a workplace located at 4526 Massillon Rd North Canton, Ohio 44720.
