On the roof at a residence, located 143 South Atlantic Avenue, Ormond Beach, FL 32173:   On or about 1/26/2022, the employer exposed employees to  eye injury hazards, in that eye protection was not utilized by employees who were operating pneumatic nail guns to secure roof shingles.

On the roof at a residence, located 143 South Atlantic Avenue, Ormond Beach, FL 32173:   On or about 1/26/2022, the employer exposed employees to fall hazards ranging from 6 to 24-feet, in that fall protection, was not utilized, while performing roofing work.
