29 CFR  1926.451(g)(4)(iv):Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface:  During an overall inspection of the facility from March 18 to 21, 2023, employees were exposed to a fall hazard in that the midrail of the northeast portion of a third-party erected supported scaffolding located in the Coker Unit blowdown area was not installed midway between the top edge of the guardrail system and the platform surface leaving a gap of more than 20 inches between the top-rail and the mid-rail as piping congestion in the area prevented the installation of the scaffold midrail at the approximate midway point.
